General comments
Aanpassen op geleide kweek.
Na 2-3 wk switch naar oraal indien mogelijk op geleide kweek.
Punctieverwonding: dosering ceftazidim evt. verlagen op basis van MIC. Tobramycine totdat resistentiepatroon bekend is.
•Flucloxacilline 1000 mg oplaaddosis iv gevolgd door 2 dd 3000 mg als continu infuus.
Aminoglycosiden: dit is gentamicine 5 mg/kg iv 1 dd, 1 week (in overleg met A-team).
Ceftazidim 1000 mg oplaaddosis iv gevolgd door 4000 mg / 24 uur als continu infuus.
