General comments
vancomycine dosering: oplaaddosis 20 mg/kg, vervolgens 35 mg/kg continu per 24 uur, OF oplaaddosis 30 mg/kg, vervolgens 17,5 mg/kg 2 dd. Onderhoudsdosering vancomycine verder afhankelijk van nierfunctie en van gemeten spiegels, zie bij vancomycine en bij TDM vancomycine
In CWZ:
flucloxacilline 1000 mg oplaaddosis gevolgd door 3000 mg 2 dd. gedurende 12 uur in continu infuus. bij ernstige infectie in combinatie met 3 dagen gentamicine (1 dd 3 mg/kg).
